How to Schedule an Appointment

Online Portal

  1. Click on the button.
  2. Log in with MyState credentials.
  3. Click on appointments.
    1. Follow the prompts to:
      1. List symptoms and phone number
      2. Choose clinic
      3. Choose appointment reason
  4. Available appointments will then show up.
  5. Click on the appointment time that will work for you.

Not all appointments are available to schedule online.
